Aircraft overturns near Monroe airport

MONROE – A single-engine piston aircraft with three people on board overturned while landing Feb. 23. The aircraft ended up about 200 to 300 feet from the runway at Charlotte-Monroe Executive Airport.

All three occupants were transported to Atrium Health Carolinas Medical Center with non-life-threatening injuries. Airport staff immediately responded and alerted city police and fire personnel, who arrived quickly to assist the airport and passengers.

The Charlotte-Monroe Executive Airport runway is closed following the incident. The runway will remain closed until Federal Aviation Administration investigators give the all-clear.

Plane veers off runway in Nuquí: seven occupants escaped injury

Tense moments were experienced at the Reyes Murillo Airport , in the municipality of Nuquí , after a plane covering the Nuquí–Quibdó route went off the runway at the moment of takeoff

According to preliminary reports, the aircraft reportedly experienced a loss of power during the maneuver, causing it to end up on the side of the runway, near the airport's safety fence.

There were seven people on board the aircraft: five passengers and two pilots, who, despite the severity of the incident, escaped injury. No one required hospitalization.

Following the incident, security protocols were activated and aeronautical authorities began the corresponding technical verifications to establish the exact causes of the incident and determine if it was due to a possible mechanical failure, runway conditions or external factors.

Air operations at the terminal were evaluated while routine inspections were carried out. The case remains under investigation.

Witnesses Observed The Pilot Attempting To Take Off From His Driveway For A Local Flight

On February 2, 2026, at about 1301 mountain standard time, an Enstrom F-28C helicopter, registration N5692Y, was substantially damaged near Leeds, Utah. The pilot was seriously injured and the passenger sustained minor injuries. At the time of the accident, the helicopter was being operated as a Title 14 Code of Federal Regulations Part 91 personal flight Witnesses observed the pilot attempting to take off from his driveway for a local flight. The helicopter lifted off the ground to a hover, which the witnesses described as “unstable,” prompting the pilot to land. Witnesses heard an increase in main rotor RPM followed by a near vertical climb. Upon reaching about 80 feet above ground level, the helicopter suddenly descended, struck a tree with its tail rotor and subsequently impacted the ground with the main rotor blades. The helicopter came to rest on its left side about 40 feet from its departure point. During the accident sequence, the main rotor, tail rotor, landing gear and fuselage were substantially damaged.

The pilot reported that he did not have sufficient power to maintain altitude and lost control of the helicopter. The helicopter was recovered to a secure location for further examination.

Narrative:
China Southwest Airlines flight 4509, a Tupolev Tu-154, crashed following a loss of control while on approach to Wenzhou Airport, China, killing all 61 occupants.

Flight SZ4509, a Tupolev Tu-154M, departed Chengdu Airport in China at 14:35 hours on a domestic flight to Wenzhou. The en route altitude was 11400 meters.
At 16:02 hours, when the aircraft passed 9000 meters in the descent towards Wenzhou, the crew felt that the steering column was too far forward. The crew were puzzled by the control issue, especially at 16:26 hours when the steering column was fully forward. Yet, the aircraft was not in a nose-down attitude. The captain noted that the controls felt disconnected. At 16:29 the flight was cleared to descend from 1200 m to 700 m. When the flight crew selected the first stage of flaps, an angle-of-attack warning sounded in the cockpit. The aircraft entered a stall and began a steep descent until it impacted a farm field at Rui'an.
Several farmers were injured by the flying debris.

PROBABLE CAUSE: A self-locking nut, other than castle nut with cotter pin as specified, had been installed at the bolt for connection between pull rod and bellcranck in the elevator control system. The nut screwed off, resulting in bolt loss, which led to the loss of pitch control.
