Here are the stories from over the weekend...

Of note, a few stories that involve aircraft on fire, one in particular involving an agricultural aircraft (crop duster) used to spray crops with fertilizer, pesticides or other chemicals. All of that smoke you see is full of crap that can mess you up, maybe even kill you. Wear your full PPE, including your SCBA and give yourselves that better chance to stay healthy and live long enough to enjoy seeing your families grow old and to collect a lot of money from your pension/retirement funds!

Be safe out there!

Tom Tiffany's plane crashes during emergency water landing after campaign event

The Republican candidate for governor 'exited the aircraft and swam to a shallow area' according to a campaign statement

Matt Smith

The plane carrying Republican candidate for governor Tom Tiffany was forced to make an emergency water landing near Wausau Saturday night after the plane lost power, according to a statement from the Tiffany campaign.

"After landing, we called 911," Tiffany said in a statement. "As the plane began to submerge, we exited the aircraft and swam to a shallow area, where we waited until emergency responders reached us by boat."

Tiffany was returning home from the La Crosse County Lincoln Day Dinner. Tiffany said the plane, described as a four-seat Bonanza, lost power while approaching the Wausau Downtown Airport.

The Marathon County Sheriff's Office said the initial 911 call came in at 8:49 p.m. Saturday. The sheriff's office identified the pilot as 73-year-old Leonard Boltz from Pearson.

Both the Federal Aviation Administration and National Transportation Safety Board are investigating.

"Thank you to everyone across Wisconsin and around the country who has reached out with prayers and kind words," Tiffany said Sunday morning. "I'm especially grateful to the pilot, the Wausau Fire Department, law enforcement, first responders, and the team at Aspirus Wausau Hospital for their incredible care and quick response."

In a new statement this morning, Tiffany's campaign said he received stitches above his right eye at Aspirus Wausau Hospital overnight and was released. The pilot was also released.

Democratic candidate for governor David Crowley said in response, "I’m grateful to God that Congressman Tiffany and no one else was hurt in this horrifying incident, which could have been a tragedy if not for the heroism of the pilot. I wish them and the Congressman a quick recovery and that they can find solace with their loved ones."

Tom Tiffany issued the following statement after the plane carrying him home from tonight's La Crosse County Lincoln Day Dinner made an emergency water landing in Lake Wausau:

"While returning home from the La Crosse County Lincoln Day Dinner, the plane I was traveling in lost power as we approached the Wausau Downtown Airport. The pilot made an emergency water landing in Lake Wausau.

"After landing, we called 911. As the plane began to submerge, we exited the aircraft and swam to a shallow area, where we waited until emergency responders reached us by boat.

"I want to express my deepest gratitude to the pilot, the Wausau Fire Department, and all of the first responders who came to our aid so quickly. They got us safely off the water and transported us to Aspirus Wausau Hospital, where we received excellent care. Thankfully, both the pilot and I sustained only minor injuries.

"God was watching over us tonight, and I am incredibly grateful to be here in the hospital with my wife, Chris, by my side. An experience like this reminds you just how precious life is."

Aircraft: Staudacher Jon ZZ 102 Injuries: 2 Serious
Flight Conducted Under: Part 91: General aviation - Personal

On August 12, 2026, about 1906 eastern daylight time, a Staudacher ZZ 102 experimental airplane, N134ZZ, was substantially damaged when it was involved in an accident near Vicksburg, Michigan. The airline transport pilot and one passenger were seriously injured. The flight operated under the provisions of Title 14 Code of Federal Regulations Part 91 as a personal flight.

Preliminary information provided by law enforcement indicated the airplane departed from Thrall Lake Airport (7MI3), Vicksburg, Michigan, and climbed to about 1,500 ft when it descended and impacted terrain. Both occupants were airlifted to hospitals, and the airplane sustained substantial damage to the fuselage and both wings.

According to the FAA aircraft records, the airplane was likely a unique build with only the accident airplane currently registered. The airplane was built in 2007 and purchased by the pilot in October 2021. According to a Barnstormers Facebook listing, dated November 8, 2025, the airplane had 210 hours total time, and the engine had 210 hours since major overhaul.

Postaccident examination by FAA inspectors revealed the presence of fuel in both fuel tanks. However, when draining fuel from the fuel tanks, an orange substance was found in the fuel line. The airplane was retained for further examination.

(Pilot) Extended The Wing Flaps In Preparation For Landing And Subsequently Heard A Loud “Pop”

Location: Salmon, ID Accident Number: WPR26LA302
Date & Time: August 9, 2026, 14:20 Local Registration: N247JP
Aircraft: Cessna 208B Injuries: 9 None
Flight Conducted Under: Part 135: Air taxi & commuter - Non-scheduled

On August 9, 2026, at 1420 mountain daylight time, a Cessna 208B airplane, N247JP, was substantially damaged when it was involved in an accident near Salmon, Idaho. The pilot and eight passengers were not injured. The airplane was operated as a Title 14 Code of Federal Regulations (CFR) Part 135 ondemand air taxi flight.

The pilot reported that, after an uneventful flight from the departure airport, he joined the downwind leg of the traffic pattern for runway 17. While turning onto the base leg, he extended the wing flaps in preparation for landing and subsequently heard a loud “pop.” Immediately thereafter, the airplane made an aggressive, uncommanded roll to the left and banked to about 45°. The pilot attempted to regain lateral control by applying right aileron. He moved the control yoke to the full right aileron position and adjusted the aileron trim to the full right setting. The airplane touched down normally, and the pilot taxied it to the ramp without further incident.

Following the landing, a company mechanic examined the flight control systems and found substantial damage to the left wing spar. A preliminary examination identified a loose flap bolt that allowed the lower portion of the aft bellcrank to move freely. The aft bellcrank was designed to attach to the airplane with a bolt secured to an anchor nut assembly on its lower surface. The hardware will be retained for further examination.

Passenger Reported That Oil Spray Was Coming In The Cockpit And He Could Smell Burning Oil

Location: Bogue Chitto, MS Accident Number: CEN26LA313
Date & Time: September 8, 2026, 11:00 Local Registration: N680HW
Aircraft: Stearman A75 Injuries: 1 Minor, 1 None
Flight Conducted Under: Part 91: General aviation - Personal

On September 8, 2026, about 1100 central daylight time, a Boeing-Stearman A75L300 airplane, N680HW, was substantially damaged when it was involved in an accident near Bogue Chitto, Mississippi. The pilot sustained minor injuries and the passenger was uninjured. The airplane operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot reported that in cruise flight, the passenger reported that oil spray was coming in the cockpit and he could smell burning oil from the engine. The pilot saw that oil pressure had decreased and began to divert to the nearest airfield. When the oil pressure read zero, the pilot shut the engine down and performed a forced landing on a road. During the landing, the airplane’s lower right wing collided with a road sign, and the pilot subsequently lost control of the airplane. The airplane then turned and veered through a fence and came to rest in a pasture. During the accident sequence, the fuselage and the upper and lower wings sustained substantial damage.

A visual inspection of the engine by responding FAA inspectors did not locate an obvious source for the reported loss of oil. The airplane was retained for further examination.

Narrative:
Aeroflot-Nord flight SU821, a Boeing 737-500, crashed following a loss of control on approach to Perm Airport, Russia, killing all 88 occupants.

Aeroflot-Nord flight 821 took off from Moscow's Sheremetyevo Airport at 21:13 hours on a domestic flight to Perm, Russia. Contact with the flight was lost while the airplane was at 3600 feet in "difficult weather conditions", according to Aeroflot officials.
The plane came down on the outskirts of the city, hitting the ground just a few hundred yards from small wooden houses and apartment buildings.
Investigation revealed that the pilot lost spatial orientation during the night-time approach through clouds. This led to a banking of the plane onto its left wing, and its entering into an intensive descent and collision with the ground. The pilot was not familiar with the attitude indicator (ADI) used on Western jets, compared to those of Russian jets. Also, an unspecified amount of alcohol was detected in the pilot's body, and he was overworked.

The immediate cause of the accident was spatial disorientation of the crew, especially the Captain who was the pilot flying at the final stage of the flight, which led to the left flip-over, a steep descent and the crash of the aircraft. The spatial disorientation was experienced during the night time operation in clouds, with both autopilot and autothrottle disengaged. Contributing to the development of the spatial disorientation and failure to recover from it was a lack of proficiency in aircraft handling, crew resource management and of skills associated with upset recovery using "western"-type attitude indications that are found on foreign and modern Russian-made aircraft. This type of indication differs from the one used on aircraft types previously flown by the crew (Tupolev 134, Antonov 2).
The systemic cause of the accident was insufficient management by the airline of flight and maintenance operations of the Boeing 737 type of aircraft. These deficiencies in the aircraft maintenance also revealed through safety inspections conducted by the Russian Transport Oversight Authority and the Russian CAA after the accident.
Deficiencies in the aircraft maintenance led to a situation when flights were performed for a long time with a throttle stagger [difference in trim of right and left engines, when matching throttle position leads to a split in engine thrust while matching thrust leads to different throttle position] that exceeded the limitations in the AMM and when the maintenance staff did not follow the AMM recommended troubleshooting procedures. The need to manage the throttle stagger during the approach increased crew workload.
The forensic medical examination performed in the State Healthcare Center of Special Status “Perm Regional Forensic Expertise Bureau” confirmed the presence of ethyl alcohol in the Captain’s body before his death. The captain’s recent work schedule during the time period before the accident was conducive to fatigue and did not comply with national regulations.
